Heights Home Health was established in 2004 with the mission to assist persons, of the community, in achieving and maintaining their desired quality of life, by providing a high caliber, community focused home health system.
Heights Home Health is a full-service, Medicare/Medicaid certified agency that provides home care for patients with varied needs and health problems. Our comprehensive services include patient care as well as teaching in the home.
To qualify for payment thru Medicare you must be homebound. As defined by Medicare, a patient is considered homebound when it takes a considerable or taxing effort for the patient to leave the home.
Generally speaking, a patient will be considered homebound if he/she has a condition due to an illness or injury that restricts his/her ability to leave his/her place of residence except with the aid of supportive devices such as crutches, canes, wheelchairs and walkers, the use of special transportation, the assistance of another person or if leaving the home is medically contraindicated.
In most instances a "homebound" patient's absence from the home will occur for the purpose of receiving medical treatment.
Absences should be infrequent, of relatively short duration and do not indicate that the patient has the capacity to obtain health care provided outside rather than inside the home.
Any absence for religious services is deemed to be an absence of infrequent or short duration, and thus does not negate homebound status.
